Serious bad breath

Am having serious bad breath....ave tried non alcoholic mouthwash..oral hygiene but all the same..i feel like is something at the right side of my throat But what I noticed is that sometimes if I try to brush after eating little will come up to my mouth from that same right side of the throat...could it be there is something wrong there...bcos same thing happened just now and along with the food there is a little cotton wool there which comes up.....
