From whatever little you have told here, he seems to be suffering from a psychotic disorder. But a detailed assessment needs to be done and a diagnosis made following which the right treatment will be started. Psychosis has good remission rates with medicines.

Suspiciousness, believing that everyone is against him, feeling as if people are talking about him and following him, hallucinations can occur in mental health disorders like psychosis.
