It sounds like you are going through an incredibly stressful and difficult situation trying to manage your mother's care. However, you should not restart her on Amazeo 200mg (Amisulpride) without a psychiatrist's direct evaluation. When a patient restarts an antipsychotic medication after a long break, it is critical to evaluate their current physical health (including blood pressure, cardiac profile, and metabolic factors) and titrate the dose properly. Starting immediately with a dose like 200mg without a gradual taper up can lead to severe side effects. Furthermore, her current symptoms—responding to internal stimuli (answering voice hallucinations), persecutory beliefs about returning home, and mood swings—indicate an active relapse that requires a clinical assessment to determine if Amisulpride is still the correct molecule or if her treatment regimen needs to be modified.
Next Steps
Consult her treating psychiatrist immediately: Schedule an urgent physical or teleconsultation appointment. Because she is currently paranoid about returning home, a teleconsultation might be an easier first step to get her evaluated without causing her further distress. You can book a session with me as well for the above. Do not force her to return home immediately: If she associates the house with the onset of distressing hallucinations, forcing her back may increase her agitation and cause her to lose trust in you. Maintain a calm, non-confrontational environment wherever she currently is. Acknowledge her distress, not the hallucinations: When she talks about the voices or fears, do not argue with her or try to prove her wrong. Instead, validate her feelings by saying something supportive, like, "I understand that you feel scared right now, and I want to help you feel safe."
Health Tips
Emergency Monitor: Watch closely for signs of severe relapse or psychiatric emergencies, such as completely refusing food and water, expressing ideas of self-harm, or becoming physically aggressive. If these occur, she will require immediate evaluation at a hospital emergency psychiatric ward. Watch for Extrapyramidal Side Effects (EPS): If a doctor does advise restarting her medication, look out for sudden muscle stiffness, tremors, or severe restlessness (akathisia), which can happen when restarting antipsychotics after a break.
