Runnung ear with foul smell, doctor recommended operation. Picture has been attached given by doctor. Please let me know is there any other option expect operation .

Ear discharge with a perforated tympanic membranes will require some antibiotics (oral and drops) with decongestant and pain medications. However if the perforation doesn’t close then surgery is the next step. The hearing loss if any might also improve.
