The confusion you’re facing is very common because propranolol comes in two completely different formulations, and each behaves differently in the body. Let me explain this clearly: 1. Sustained-release (SR) vs Immediate-release (IR) propranolol SR 20 mg releases the medicine slowly over 24 hours. IR 10 mg works fast but wears off in a few hours. So switching from SR → half SR → IR without proper medical guidance will always give fluctuating results, because the timing, duration, and dose strength change completely. This is why you’re feeling the effect wearing off quickly. 2. You should NOT adjust the dose on your own Propranolol dosing depends on: Your baseline heart rate Your BP pattern Your anxiety/stress triggers Your sleep pattern Any other medicines you’re taking Your lifestyle and work routine If the dose is changed wrongly, it can cause: Too-low heart rate Sudden rebound palpitations Increased fatigue Dizziness or low BP Worsening anxiety episodes This is why dosing must be planned case-by-case, not generically. 3. Your symptoms tell me the issue is not the medicine it’s the timing and pattern of usage Before deciding: whether you need IR or SR how many times per day what strength is actually correct for you I need to understand: When do your palpitations start? What is your usual resting heart rate? How long have you been taking propranolol? Did you stop abruptly anytime? Do you have anxiety episodes or panic sensations? Do symptoms worsen at a particular time of day? These details determine the correct approach. 4. What you should do next Instead of changing tablets on your own, you need a proper review of your symptoms + reports + current pattern of palpitations. Once I evaluate these, I will guide you clearly on: Whether SR or IR is suitable for you What timing will control your symptoms best Whether the current dose needs adjustment How to avoid rebound palpitations Whether another underlying cause needs to be checked This cannot be done safely through a public answer without full evaluation. If you want, I can review your complete case privately Propranolol dosing is very individual, and the safest approach is to let me review: Your symptoms Your heart rate pattern Stress/anxiety triggers Your previous medication schedule You can book a private consultation with me on whatsapp at Nine Three Two Six Zero Two Zero Five Three Six, and I’ll help you decide the correct plan so your palpitations stay fully controlled without trial-and-error.

✅ 1. Why cutting a Sustained-Release (SR) tablet is not advised Propranolol SR/LA/XL tablets are designed to release the medicine slowly over 24 hours. If you break them: The slow-release mechanism gets damaged The dose may release too quickly Effect becomes unpredictable Side effects may increase So you were correct to stop cutting the SR 20 mg tablet. ✅ 2. Why 10 mg Immediate-Release (IR) once daily is not helping IR propranolol lasts only 4–6 hours. Taking it once daily means it stops working for most of the day. That’s why you are again getting pulse/palpitations. ⭐ Your two safe options (discuss with your doctor): Option A: Take Propranolol Immediate-Release twice or thrice daily Typical dosing pattern is: 10 mg IR 2–3 times per day (every 8 hours) This keeps effect stable throughout the day. Option B: Switch back to Propranolol SR 20 mg once daily This is the easiest and smoothest option for most patients because: One dose per day Effect lasts full 24 hours Better for anxiety-related palpitations No fluctuations If 20 mg SR is too strong or weak, your doctor can adjust to 40 mg SR or 10 mg IR multiple times, depending on symptoms. ❗Important Safety Points Do not adjust dose on your own—always consult a doctor. Never stop propranolol suddenly → it may cause rebound high heart rate. If you have asthma, very low BP, dizziness, or severe fatigue, propranolol may not be suitable. 📌 My suggestion (safest general direction): Since once-daily IR is not enough, you have two reasonable choices: ➡️ Take 10 mg IR twice daily (common regimen) OR ➡️ Use SR 20 mg once daily for steady control But final decision must be made by your prescribing doctor. If you tell me: Your average pulse rate BP readings Whether symptoms are from anxiety or thyroid or heart …I can guide you even more precisely.

If the effect wears off after a few hours,  you need a change in treatment. You can take propanol 10mg immediate release twice, but you should consult a cardiologist before. It has side effects and is not for everyone.
Next Steps
Consult a cardiologist.
