Regarding MCT oil use in newborn milk
Hello Doctor, My baby was born preterm at 34 weeks 3 days on 4th December at 9:00 PM. Current age is around 3–4 weeks and current weight is ~2.2 kg (lowest was 2.0 kg). Our pediatrician has prescribed MCT oil – 10 drops, to be mixed with milk. Baby is being fed mostly expressed breast milk by bowl/spoon, with total intake around 300 ml in 24 hours. I would like to confirm: Is MCT oil safe for preterm babies? Should it be given with every feed or selected feeds only? For how long is it usually continued? What side effects should we monitor (loose stools, spit-ups, gas)? How do we know when to stop it? Please advise. Thank you.

Dear Parent, There is no doubt that MCT oil is safe when given in the correct small dose. Your baby was born at 34 weeks and is now doing well at 2.2 kg. Our main focus should be on exclusive breast milk, which is the best and most complete food for growth. To help with better weight gain: • Always express the breast completely before or after each feed. • The hindmilk (the thicker milk that comes at the end) is rich in fats and calories — this is especially helpful for weight gain. • Try to ensure your baby gets plenty of this hindmilk. Many late preterm babies around 2–2.5 kg gain weight very well on exclusive breast milk alone. About MCT oil (10 drops ≈ 0.5 mL per feed): • This small amount with every feed is safe and suitable because your baby’s weight and progress are good. • It simply provides a little extra easy-to-digest fat to support faster weight gain in these early weeks. • At this normal low dose, there are no expected side effects. Although we often use MCT oil very cautiously in very low birth weight babies (2 kg). How long to continue MCT oil? We will continue the current dose until your baby shows steady proper weight gain, with or without MCT oil — usually until around 2.5kg After that, if growth is good, we can reduce or stop it and continue with exclusive breast milk only. Tips to improve feeding and milk supply: • Encourage non-nutritive sucking (comfort sucking at the breast even when the baby is not hungry). • This helps accelerate the let-down reflex, so the baby gets hindmilk earlier and feeds more effectively. • Frequent non-nutritive sucking also stimulates more milk production. • After each feed, express any remaining milk to fully empty the breast — this tells your body to make more milk. You are doing an excellent job. With plenty of breast milk (especially hindmilk) and the small temporary support from MCT oil, your baby will continue to grow beautifully.
