Puss coming from penis

Hi doctor how r u? I had piles since 10 years without treatment.some time it's been very severe pain in my infected place.now some days before I feel irritating in my penis and feel pain when i pass urine .today I saw a fungus or puss type white clot come from my penis when I try to pass stool. and my urine color is dark orange..normaly I have white colored urine.after that I feel some better about irritating. guide me what should I do.thx.
