Psoas muscle injury
Suffering badly for 2 years after lifting weight. The psoas muscle gets inflamed often. Mid to lower back hurts, Pain in hips, groin, and lower back on both sides. Spinal curvature seems to be more flat. MrI shows chronic sacrolitis as well but was later ruled out for arthritis since I did not respond to both Dmards, biologics, and even steroid into SI joint didn't reduce pain. Pain extremely worse while sitting or standing but feels slightly better lying down with the upper trunk lifted or when walking. MRI shows significant fat deposit near the hips and si joints that are pressing and causing pain ig. There is a burning irritation right from the mid bad to lower back on immediate left of spine (possibly psoas). While on the right there is some inflammation in the psoas muscle. Have been regularly doing lower back exercises for 6 months but not helping much. Quality of life extremely low - not able to sit for 20 mins without pain. Can someone help on what course to take now please?

a case of ankylosing spondylitis, if above therapy has not worked then injectible immunomodulators are option for you consutl a rheumatologist regarding MABs.

Hello good evening. B/L sacroilitis in a young male is suggestive of an inflammatory arthropathy. I can understand your worry and suffering. The role of medications is very very minimal in condition like these.
Next Steps
If the pain is very severe you can take anti-inflammatory pain medication temporarily. But in the long term you need to modify your lifestyle 1) physiotherapy and exercises is must 2) try swimming- atleast 3-4times per week 3) get full allergic panel test- avoid food which causes any sort of inflammation 4) keep yourself warm- cool weather will increase pain

You need to accept the condition you have. Keep doing exercises and when ever the pain episodes increase, don’t hesitate to take a course of medication Consult a good rheumatologist
