Symptoms suggest anxiety-related overactive bladder / psychosomatic bladder dysfunction, especially with long-standing anxiety and IBS and normal USG. What to do: • Joint care with urologist + psychiatrist • Treat anxiety (CBT, stress management, ± SSRIs/SNRIs which also help IBS & bladder sensitivity) • Bladder training + pelvic floor exercises • Avoid caffeine, nicotine; timed voiding • If anticholinergics not tolerated, discuss β3-agonist (mirabegron) with urologist Treating anxiety often significantly reduces urinary urgency and leakage.
