Usually severe pain is seen in anal fissure. You should take sitz bath (Hot water tub bath). Take some mild laxative Ayurvedic medicine like Sushruta Churna/ Haritaki Churna 1 tea spoon full for bowel regulation. Application some soothing ointment like Sushruta ointment/ Coconut oil/ cow's ghee in anal passage 3/4 times a day. For further assistance please call reception.

Got ur pain n concern though it's unfortunate but it's result of long standing stomach upset n constipation.
As u shared internal piles n fissures it could have shared by ur treating dr so follow him for seitz bath by local application n have lots of warm Buttermilk
