Pain likely due to piles + constipation. Take plenty of water, high-fiber diet, and a stool softener like lactulose at night. For pain, warm sitz bath 2–3 times daily and apply lignocaine gel locally. If severe pain or bleeding continues, consult me for proper medicines and examination.

Hello. I have reviewed your concerns regarding the pain and constipation you are experiencing. The pain you described, especially it lasting for up to 2 hours after using the washroom, is often caused by a small tear in the lining of the anus (an anal fissure) or inflamed internal/external hemorrhoids (piles). This cycle is usually triggered and made worse by constipation and straining. To help me give you better advice: Have you noticed any bright red blood on the tissue or in the commode? Do you feel a lump or swelling in the area? Immediate Pain Relief Sitz Bath: This is the most effective immediate remedy. Sit in a tub of plain warm water for 10–15 minutes, 2–3 times a day. This helps relax the anal sphincter muscle and significantly reduces pain. Topical Ointments: You can use an over-the-counter soothing cream containing a local anesthetic (like Lidocaine) and a calcium channel blocker (like Diltiazem) if a fissure is suspected. Apply this about 20 minutes before passing stool and after the Sitz bath. Managing Constipation (Long-term) Fiber and Water: You must aim for 25–30 grams of fiber daily (oats, whole grains, leafy vegetables) and drink at least 3 liters of water. Fiber only works if there is enough water to move it through the system. Stool Softeners: Since you have active pain, you may benefit from a mild osmotic laxative (like Lactulose or Polyethylene Glycol) at night to ensure the stool is very soft the next morning. This prevents further tearing or irritation. Correct Toilet Posture: Using a small footstool to elevate your knees while sitting on the toilet can help straighten the rectum and make evacuation easier without straining. Recommended Next Steps I recommend a physical examination (Digital Rectal Examination) by a general surgeon or a physician to confirm if this is a simple pile or an anal fissure, as the treatment for a fissure specifically focuses on relaxing the muscle. Seek medical attention immediately if you experience: Heavy, persistent bleeding. Severe, unbearable pain that prevents you from sitting or walking. Fever or discharge from the area.

Symptoms are suggestive of Hemorrhoids with a possibility of associated Anal fissure (since pain is lasting for a couple of hours after passing stool). For now you can start: * Syp Duphalac 15 ml at bedtime for 5–7 days (adjust dose to keep stools soft) * Ointment Anobliss Ointment apply locally 2–3 times daily, especially before and after passing stool * Warm sitz bath 10–15 minutes, 2–3 times daily * High fiber diet (fruits, vegetables) + adequate water intake Avoid straining and prolonged sitting during defecation. This should give symptomatic relief in a few days. However, as pain is significant and prolonged, exact cause (piles vs fissure) may need confirmation and treatment adjustment accordingly. If symptoms persist beyond 3–5 days, worsen, or if there is bleeding, a follow-up consultation is recommended for further evaluation and management.

This looks like a problem of external hemorrhoids. There could be associated anal fissures also. constipation should be treated as it forms a vicious cycle with the piles
Next Steps
Consult a doctor preferably a general surgeon to exam the area and plan the treatment
Health Tips
Fibre rich food, plenty of water, sitz bath, Lactulose/Isagol can be used for 2-3 weeks until the fissure/pain decreases

First diagnose is it piles Or not. Piles causes flesh like mass coming out and in Independently or with some action by you. Probably you're having fissure. What I suggest : 1. Plenty of fibers(it'll loosen and form your stool) 2. Plenty of water(3-4L/day) 3. Sitz bath(take bucket where you can sit. Fill half of it with Luke warm water and add 3 scoop betadine) sit for 10-15 minutes twice a day for 10 days
Next Steps
if it's not resolving within 3-5 days. you can contact me for any medication if needed. Because it might require examination to rule out any infection and avoid unnecessary tests.
Health Tips
don't eat junk food, less fiber food in the mean time. don't eat fibers if you're having SIBO/IBD/IBS

"The symptoms you are describing—sharp pain during evacuation that lasts for up to 2 hours—suggest that in addition to Piles (Hemorrhoids), you may have developed an Anal Fissure (a small tear in the lining of the anus). This is very common when chronic constipation causes the passage of hard stools. The 2-hour lingering pain is typically due to a 'sphincter spasm,' where the muscles of the anus tighten involuntarily after being irritated by hard stool

Health Tips
Avoid sitting on the toilet for long periods and avoid straining. If you notice bright red blood in the stool or if the pain becomes unbearable, a physical examination by a General Surgeon is necessary to rule out a perianal abscess. For a detailed clinical evaluation, a specific medication prescription for pain and constipation, and direct follow-up, you can consult me directly through my profile."

Istly understand, that constipation will put stress on the bowel and make evacuation of bowel tough. And the more you will fear evacuation harder the stool will get and cause more constipation. 1. A stools softner like lactulose can be taken orally. 2. Can do sitz bath( take providone iodine solution, dissolve in warm water, put all of this in a tub ) and  sit in it three times a day. 3. Locally lignocaine jelly can be applied.
Next Steps
should visit and get it examined
