It’s very difficult to comprehend from the pictures you provided. However, the one in front towards the scrotal end look like a fissure. If it’s painful with no or minimal bleed and pain exacerbating during passage of stools it is most probably a fissure or fissure with an abscess. Piles is really cause painless bleed
