My wife has a perforated septum. None of the usual reasons for the perforation apply to her. She's not a drug user, nor prior surgeries. No injuries to the nose. Not taking any medication. Randomly her nose starts whistling and there's a small hole. We are scheduled to have it fixed by taking tissue from another part of her nose and covering the hole with it. Hello, I must say you have made a very good question and you have provided almost the details one would require...You have stated all the common causes and when these are not there we look for the unusual causes.... Many times when there are no sign of inflammation then one can plan surgery and during surgery itself a biopsy can be taken. The procedure is a reconstructive one so there always remains a chance of recurrence... What we do is , we endoscopically advance a flap from the nose inner lining itself to cover the hole in the septum... That avoids an incision elsewhere in the body...
