Pangastritis small hiatus hernia
Hello Dr I have epigastric middle area cramps bloating pain indigestion acid reflux issues. Taking dexlansoprazole 60 and cizaspa x but no relief. 6 months before in September 2025 tested endoscopy detected pangastritis & small hiatus hernia. I want to ask that are these issues because of small hiatus hernia. And 4 days before I had consulted gi surgeon for this he suggested me to go for hiatus hernia surgery with nissen fundoplication. I want to ask that should I go for a surgery. Endoscopy report attached

Though Nissen's fundoplication is the ultimate solution, there are complications associated with it like indigestion , nausea etc. So, consider this treatment carefully only  if your symptoms are worse and intolerable.

medication can be changed and tried if symptmatic improvement is seen or not, newer generation medication make surgery a later choice of action. please consult online for better history and opinion

Hi get H. Pylori testing done if not done already and treat accordingly if positive, can also try changing the proton pump inhibitors and lastly get tested for Gerd by getting 24 hour ph monitoring done and rule out achalasia by getting manometry done. Lifestyle changes to be done - like reduce coffee, less  spicy food, weight loss. If everything is tried and proven reflux and no improvement in symptoms - surgery is the last option would be Fundoplication (toupe/dor) with hiatus hernia repair. Surgery comes with minimal complications and permanent solution. Done laparoscopically these days so early recovery is guaranteed.

Hello As medication is not helping and the symptoms are persistent, as your GI surgeon told you, you should consider the surgical treatment as well. You should plan on doing lifestyle changes more than the Surgery. You can consult with your GI Surgeon again, mainly lifestyle changes like eating on time, type of food, you should consume, no tea or coffee, sleeping early and walking after meals because only medication will not improve your symptoms. Speak to your GI Surgeon regarding surgery Vs conservative management, but if symptoms do not improve after four weeks of the treatment with lifestyle changes, you can go ahead with this procedure. And I would suggest to go ahead with a repeat Endoscopy before planning for any other procedure
