At 78 years of age with long-standing knee arthritis it is common for ultracet tramadol paracetamol to lose effectiveness over time however because you are also a BP blood pressure patient strong painkillers like long-term NSAIDs are not safe to take regularly without supervision. For long-term use, plain paracetamol within safe daily limits is the safest base option and pain control can be optimized with topical pain gels, knee support and activity modification rather than stronger tablets. If pain is severe and limiting daily activities options like short supervised stronger analgesics or joint directed treatments may be considered after evaluation i strongly recommend an orthopedic review with a knee X-ray to assess arthritis severity so pain can be managed safely without risking BP, kidney, or stomach complications.

A very good evening ma’am, pain medications have both benefits and side effects(Boon&Bane). Knee arthritis needs to be evaluated by a trained Joint Replacement Surgeon who can properly lay down your next course of action. I would recommend you to not rely too much on Pain meds and focus on the causative agent.
Health Tips
Too Much Paracetamol or Any other pain meds when taken for a prolonged duration can have side effects on your Kidneys. Take care. Happy to Help.
