The skin shows what we call as ‘peau d’orange’ and with your high eosinophils in the blood and fever, this is almost certainly’Filariasis’. Do the following, 1. Hetrazan 100mg tablets to be taken 3 times a day with food for at least four weeks 2. Continue with your antibiotics 3. Blood test for Filariasis. 4. In a week blood tests- CBP, LFT (monitoring while on Hetrazan). Don’t delay starting treatment. All the best. www.jgsr-health-education.in

Most likely you have an abscess in the anal region. If it is relapsing, consult a gen surgeon for incision and drainage.

Your symptoms — pain between the upper buttocks, redness, swelling, lump, and recurrent fever with elevated CRP — strongly suggest a local infection or abscess in the sacrococcygeal area. The most common condition causing this is Pilonidal Sinus, where a small infected tract or cyst forms in the cleft between the buttocks. This condition can cause: • Painful swelling or lump near the tailbone • Redness and warmth of skin • Fever when infection increases • Temporary relief with antibiotics but symptoms returning Antibiotics and sitz bath can reduce inflammation temporarily, but if an abscess or sinus tract is present, it often requires minor surgical treatment for complete cure. Your CBC report appears mostly within normal limits except mild eosinophilia, which is not directly related to this infection. The raised CRP supports that there is active inflammation or infection.
Next Steps
You should consult a **General Surgery specialist or Colorectal Surgery specialist. They will examine the area and may advise: • Ultrasound of the local area if needed • Incision and drainage if there is an abscess • Definitive surgery for pilonidal sinus if a sinus tract is present Continue until consultation: • Warm sitz bath 3–4 times daily • Keep the area clean and dry • Avoid prolonged sitting If swelling becomes very painful, pus starts discharging, or fever rises, you should seek urgent surgical evaluation.
Health Tips
• Maintain good hygiene of the buttock cleft area. • Avoid tight clothing and prolonged sitting. • Hair removal in that region may sometimes be advised after treatment to reduce recurrence. • Early treatment prevents recurrent infections or chronic sinus formation. ⸻ If you want, you can also tell me whether the lump is exactly in the midline near the tailbone and whether any pus discharge is present, and I can help you confirm whether it is likely pilonidal sinus and guide you about treatment before you see the surgeon.

Hello. Pain, redness, swelling and a lump in the upper buttock/sacral area with fever can sometimes occur due to a pilonidal infection or abscess. Since symptoms improve temporarily with antibiotics but then return, it may require proper examination and sometimes minor surgical treatment or drainage. You may consider consulting a General Surgeon for evaluation. Continue sitz baths and keep the area clean and dry. Since treatment depends on examination findings, a detailed review would be helpful. You may book an online consultation for proper evaluation and personalized treatment guidance.
