Hi, antibiotic course for 21 days is very unusual. For such severe infection to occur there should also be some underlying problem which should also be addressed along with treating the infection. Either the culture report was wrong or the antibiotics that were given when were not adequate.
Next Steps
1.Ask your treating doctor for the reason behind the fever and the source of infection. Sometimes non infectious source of fever do occur such as antibiotics themselves can cause fever.
2. Find is adequate nutrition is provided or not, because most elderly are malnourished and infections warrants good nutritional support for rapid recovery.
3. Ask your treating doctor what is the underlying problem that actually caused her uti. Is diabetes or any other health related issue that makes her prone for severe infections.
4. Always ask and clarify the evidences based on which the doctor is making decisions. If he is unable to provide you evidences and guidelines then he is not a capable doctor.
5. If things do not seem to settle consult a doctor specialised in infectious diseases.
