Hi,
It sounds like you're experiencing severe emotional distress, including intense OCD, paranoia, intrusive thoughts, hallucinations, and thoughts of self-harm or harm to others, which may indicate a complex mental health condition possibly involving psychosis or a severe mood disorder. Your history of trauma, family issues, and past abuse can contribute to these symptoms. It's crucial to seek urgent help from a mental health professional, such as a psychiatrist or therapist, who can assess your condition thoroughly and provide appropriate treatment, which may include therapy, medication, or both. You're not alone, and professional support is essential to help you manage these symptoms, ensure your safety, and work towards recovery.

What you've described is very serious and needs professional attention as soon as possible.
You're dealing with multiple stressors at once:
A history of OCD and severe anxiety
Past sexual abuse (SA) and family difficulties
Significant physical illness (TB)
Relationship-related fears and jealousy
Hearing voices/noises that tell you people will leave you
Suicidal thoughts and homicidal thoughts
Persistent fantasies about death and increasing hatred
The fear of losing your boyfriend, repeatedly seeing other women as threats, checking, ruminating, and intrusive thoughts can occur in OCD. However, the hearing voices/noises, increasing hostility, suicidal thoughts, and homicidal thoughts require urgent assessment because they may indicate that something more than OCD is happening or that your current symptoms have become much more severe.
It is not possible to diagnose you online, but these symptoms should not be ignored.
Next Steps
Please contact a psychiatrist or mental health professional urgently, ideally within the next 24–48 hours.
Tell them openly about:
Suicidal thoughts
Homicidal thoughts
Hearing voices/noises
OCD symptoms
Past trauma and abuse
If you feel you might act on thoughts of harming yourself or someone else, seek emergency help immediately and do not stay alone.
Inform a trusted family member, partner, or close friend about what you're experiencing.
Health Tips
Do not act on urges, impulses, revenge fantasies, or thoughts of harming yourself or anyone else.
Avoid contacting, monitoring, threatening, or confronting the classmate.
Avoid making legal or relationship decisions while emotionally overwhelmed.
Try to reduce isolation and stay around trusted people.
Remind yourself that thoughts and urges are not the same as actions, but when thoughts become intense, professional support is needed.
Regarding Legal Concerns
Having a mental health condition does not automatically protect someone from legal consequences for actions they take. Legal outcomes depend on many factors and require advice from a legal professional, not a mental health forum.
Important Safety Question
Because you mentioned both suicidal and homicidal thoughts, I need to ask:
Are you currently having thoughts of acting on these feelings, or do you have any plan or intention to harm yourself or another person?
If the answer is yes, or even "maybe," please seek immediate emergency psychiatric help or contact local emergency services and a trusted person right now. Your safety and the safety of others are the priority.
What you’re describing may be more than OCD alone. Severe OCD can cause intense jealousy, threat perception, obsessive focus on a partner, intrusive aggressive thoughts, and impulsive reactions. However, hearing voices/noises, growing hatred, and the impact of past trauma and sexual abuse suggest that a psychiatrist should evaluate you to rule out other conditions as well.
One important concern is that you mentioned both suicidal and homicidal thoughts. Please don’t ignore these symptoms or try to diagnose yourself online. A proper psychiatric assessment is strongly recommended so you can receive the right treatment and support. Take therapy. You can connect with me on nine two six six seven two six zero six five.
What you’re going through right now is a lot — TB, OCD flaring, a relationship being threatened, and now these frightening thoughts and sounds. That’s not weakness. That’s a system under extreme overload.
The thoughts about death — yours or hers — and the voices you’re hearing are serious symptoms that need in-person psychiatric support urgently. This isn’t something therapy alone can manage right now. You may need a psychiatric evaluation to stabilise first.
