I can imagine it must be difficult for you.. Sleep problems can occur due to a number of factors like stress, chronic pain, Substance abuse or other mental health problems like depression,anxiety or psychosis. I can see that you are taking medications for mental heath issues. Please Consult your psychiatrist for a review of medicine. Discuss with him your sleep issues and he / she will be able to alter your medication accordingly.
You will also benefit from exercise and meditation.
