Hi Doctor, i am mother of child who raised the issue that he is facing discomfort as his foreskin retracted during puberty age and now he is facing discomfort due to it. I don't why my son don't told this to me and feeling so much shame and living with discomfort from past 4-5 years and yesterday he told me to take him to a general surgeon as advised by you. He also told me he took a consultation with urologist here he said that he have short foreskin that's why glans is not covered. I asked him if he is getting any problem in erection and show me he first denied but then i shouted on him then he showed. What i observed that his foreskin remains at same place in both erect and flaccid. I mean if it's due to short foreskin then in erection his foreskin shouldn't retracted more back ? Atd image for reference. It looks to me like his outermost tip is sticked behind the glans .I am taking him to a surgeon this evening as my husband lives abroad. Thanks to all doctors here who helped my son.

"Based on the description and the provided images, this condition is consistent with Paraphimosis that has potentially become chronic, or a severe case of a tight Frenulum Breve (a short band of tissue on the underside). When the foreskin is retracted and cannot be returned to its original position covering the glans, it can form a tight 'constricting band.' Over time, this can lead to persistent discomfort and 'sticking' of the tissue. At 18, this is a significant quality-of-life issue that requires surgical correction to restore normal anatomy and prevent future complications."
Next Steps
Immediate Surgical Review: "Your'sdecision to take him to a General Surgeon this evening is correct. The surgeon will evaluate if the tissue is truly 'stuck' (adherent) or if the constricting ring of the foreskin needs to be released."
Surgical Options: "The most definitive treatment for this condition is Circumcision. This procedure removes the tight, constricted tissue and permanently resolves the discomfort, ensuring the glans is no longer constricted during erections."
Pre-Op Counseling: "Since the son has felt a sense of 'shame' for years, it is important to reassure him that this is a very common anatomical issue that can be easily fixed with a routine 30-minute procedure."
