Need guidance before checkup
Hi Doctor, thanks for replying my yesterday query and letting me know that i am having phimosis issue as my penis foreskin stuck and not covering glans causing discomfort while walking with exposed glans. I will visit to a urologist in my town in today evening for physical checkup. But as i checked on internet it is given that full circumcision requires for phimosis treatment since then i am feeling very tensed and scared as i don't want to loose my foreskin due to personal and religious reasons. Will you please check attached images and let me know is full circumcision required in my case or it may be resolve through any cream/ointment ? Also how should i told this issue to my parents they are not educated much as we live in village that the issue i am facing is something natural because they will thought that i have done something wrong with my penis. Thankyou doctors, by showing and telling my issue with you all i gained confidence to get it checked physically by visiting urologist.

Circumcision is always a full circumcision. There is nothing known as subtotal circumcision. From the image that you have shared it seems that you are already circumcised.

Sometimes circumcision is not needed, locally applied steroid creams can help. But it is for the urologist to decide, after clinical examination and how your body responds to a local application.

If you don't want to  loose foreskin  don't go for circumcision. There is no cream  to retract foreskin.

It is completely understandable to feel tensed about surgery, but clinically, Full Circumcision is NOT the only treatment for Phimosis. Modern urology offers several conservative options, such as 'Preputioplasty' (a limited plastic surgery that preserves the foreskin) or long-term topical steroid therapy to increase skin elasticity. Regarding your concern with your parents: Phimosis is a structural medical condition, often present since birth or due to natural growth patterns; it is not caused by anything 'wrong' or 'immoral' that you have done. It is as natural as having a vision problem or a skin allergy.
Next Steps
Urologist Consultation: When you visit the urologist today, clearly state your preference for 'Foreskin-Preserving' treatments. Ask specifically about steroid creams (like Betamethasone) and whether you are a candidate for 'Dorsal Slit' or 'Preputioplasty' instead of full circumcision.. Parental Communication: If you decide to tell your parents, explain it as a 'skin tightness' issue that is causing physical discomfort while walking. Comparing it to a 'tight collar' or a 'small shoe' that needs adjustment helps simplify it for those without a medical background. Symptomatic Relief: Until your checkup, continue to wear loose-fitting clothing to minimize friction on the exposed glans, which is currently causing your discomfort while walking.
