Need Form 10-IA certificate for mental

Asked for Female, 28 Years

My sister has mental retardation and I want to claim 80DD tax claim towards her treatment. It is difficult to take her to any hospital as she fears hospital atmosphere, if there is any registered neurologist who can visit home & check her & give this form 10-IA certificate it will be great. Please let me know if possible.
