You need to do liver function tests, hba1c, serum lipid profile,  serum amylase, full blood count every 3 months. If abdominal pain or discomfort after meals, get ultrasound done to exclude gall bladder issues like cholecystitis or gall stones. Follow life style measures as well for weight reduction.

I completely understanding your concern but why r u wanting weight loss so much drastically. It's good to having 1 kg per week weight loss and you are overachieving the target. So plz don't increase the dose even I suggested decrease it from 7.5 to 5 again.

You are: • 38 years • BMI was 29.1 (overweight category) • Lost ~9.2 kg already (excellent response) • On Mounjaro (tirzepatide) • 5 mg first month → 7.5 mg second month First: That is very good weight loss. Now regarding dose escalation: Mounjaro dosing is not meant to be increased every month automatically for faster weight loss. It is titrated based on: • Tolerability • Side effects • Weight response • Metabolic parameters Standard escalation schedule (if tolerated): 2.5 mg → 5 mg → 7.5 mg → 10 mg → 12.5 mg → 15 mg But escalation is done only if needed, not just because the next higher dose exists. You are already responding well. If weight is steadily reducing on 7.5 mg, there is no mandatory need to increase further. Increasing unnecessarily may increase risk of: • Nausea • Vomiting • Pancreatitis (rare but serious) • Gallstones • Muscle loss if rapid weight drop • Excessive appetite suppression
Next Steps
✔ 1. Do NOT Increase Automatically Stay on 7.5 mg for at least: • 4–8 weeks Assess: • Weight trend • Appetite • Side effects Increase only if: • Weight plateaus for 4+ weeks • Hunger returns significantly • No side effects present ⸻ ✔ 2. Important Monitoring Tests Every 3–4 months: • Fasting blood sugarHbA1cLFT (ALT, AST) • Serum creatinineLipid profile • TSHVitamin B12Vitamin D If abdominal pain occurs: • Serum amylase/lipase If rapid weight loss: • Check for gallstones (Ultrasound) ⸻ ✔ 3. Monitor Muscle Mass Rapid weight loss can cause: • Lean muscle loss Add: • High protein intake (1–1.2 g/kg body weight) • Strength training 3–4 times/week
Health Tips
✔ Aim for 0.5–1 kg per week loss ✔ Avoid crash calorie restriction ✔ Stay hydrated ✔ Watch for persistent abdominal pain ✔ Do not chase maximum dose unnecessarily ✔ Maintain protein intake Your progress is excellent — but the goal is sustainable fat loss, not fastest weight loss. If you’d like, I can calculate your ideal target weight, safe rate of reduction, and help decide whether you should hold at 7.5 mg or escalate — a structured online review would ensure you avoid complications while maximizing results.

Not sounding like you are having this under guidance. Rapid weight loss comes with a whole lot of issues including severe protein deficiency and drastic changes to body composition requiring need to deal with overhanging skin around the torso, neck, arms and legs, usually by a plastic surgeon. Ongoing use is associated with serious stomach, pancreas, thyroid problems including thyroid cancer and others. All these are meant to be understood BEFORE starting treatment. Please at least read the information sheet that comes with the medication box. All the best. www.jgsr-health-education.in

Important tests for monitoring (every 3–4 months): • Fasting blood sugar & HbA1c Liver function test (LFT) Kidney function test (KFT) Lipid profile Thyroid profile (TSH) Vitamin B12 Vitamin D Serum amylase/lipase (if abdominal pain occurs) Important precautions: • High protein diet to prevent muscle loss• Adequate hydration• Strength exercise or walking• Avoid very oily heavy meals• Monitor for persistent abdominal pain, vomiting, or severe weakness . If you are tolerating 7.5 mg well and still losing weight, you may continue the same dose for another month before increasing. For personalized dose planning and monitoring guidance,  text me on Practo anytime.
