Keep continuing the the medication for diabetes and hypertension. For fever, do regular monitoring. The range from 99 to 100 can be normal if checked during day morning hours and early evening. So temperature charting is important. Secondly, give dolo on sos basis only if temperature is more than 100. Thirdly, follow back for online consultaion with temperature chart. Will do the needful after seeing if treatment is actually needed.

Fever in an elderly with many comorbid conditions should always be cautious as such patients don’t generate high fever or symptoms even with severe infections. So monitoring them for even smaller change in symptoms and taking necessary steps is very important, usual cause of fever in an elderly with diabetes are Urinary tract infections and respiratory infections.
Next Steps
Monitor his temperature, give dolo only after checking his temperature, if it is 100 or more. If persists consider doing a CBP, CUE. And consult a general physician.

Let him continue his usual medications.. meanwhile check his temperature and oxygen levels 6th hourly once, if temperature (underarm)is 99 degree and above it's considered as fever. If you are checking under the tongue it is 100 degree and above .. then it's safe to give dolo 650 mg tablet.. watch his symptoms.. if it doesn't subside certain blood test have to be done to know the cause of fever.drink plenty of fluids, steam inhalation and salt water gargling..
