Hi! 😊 Right lower abdominal pinching sensation with a normal ultrasound is a good sign and makes acute appendicitis less likely. It could be due to muscle strain, gas, or other common causes. A proper consultation will help evaluate it better and give you clarity. Feel free to book a slot if you'd like guidance.

A normal ultrasound makes appendicitis less likely, but doesn't completely rule it out — early or mild appendicitis can sometimes be missed on ultrasound. However, if you had appendicitis you would typically also have fever, nausea, loss of appetite, and worsening pain. If those are absent, it's reassuring. The pain being worse with breathing, walking, and movement suggests a few more likely causes: 1. Gynaecological — at 39, a right ovarian cyst or endometriosis is very common and causes exactly this kind of pinching right-sided pain that worsens with activity. A routine abdominal ultrasound often doesn't look at the ovaries in detail — a transvaginal ultrasound (TVS) gives a much clearer picture of the ovaries and uterus. 2. Musculoskeletal — a strain in the right abdominal wall muscles or an early inguinal hernia can cause pinching pain with breathing and movement. 3. Right kidney/ureter — a small kidney stone can cause right-sided pain. Get a urine routine test to check for blood in urine. Please see a gynaecologist first and ask specifically for a TVS (pelvic ultrasound). Also get a urine routine test. If your pain suddenly gets severe with fever or vomiting, go to the ER immediately.
