My father was diagnosed with Hypomania around 6 months back and is taking medication for the same since then and is feeling ok. The problem now is that he is having low sodium level i.e. 128 and low chloride 94. What medication should be given in place of Tazloc trio as it is reducing the electrolytes and what medication should be given to increase sodium and chloride.

Tazloc trio carries risk of low salt and chloride levels, particularly in elderly. In very low levels, I.V. drip is necessary, mild cases can be managed with ors powder or sodamint tablets.
For changing medicine, contact his treating physician.
Sodium 128 is low and needs attention, especially in elderly 👍
It’s often due to medicines like Tazloc trio diuretic component.
Don’t change meds on your own needs proper adjustment and monitoring. I can guide a safe plan book a quick consult 👍
"A sodium level of 128 mEq/L (Hyponatremia) and chloride of 94 mEq/L in a 79-year-old patient are significant findings that require immediate clinical attention. In geriatric patients, this electrolyte imbalance is frequently a side effect of medications like Tazloc Trio, which contains a diuretic (Chlorthalidone/Hydrochlorothiazide). Diuretics work by increasing the excretion of salt and water, which can inadvertently lead to dangerously low sodium levels, potentially causing confusion, falls, or seizures
Next Steps
Managing hyponatremia in the elderly is complex. A physical examination and a repeat electrolyte panel (Serum Sodium, Potassium, Chloride) are mandatory within 24–48 hours to ensure levels are rising safely.
