Loose motion IBS for CKD patient
I am 63 years old CKD patient with creatinine of baseline 2.5 from past 9 years. I was suffering from renal calculus initially and was diagnosed with bilateral renal calculus and was removed. Currently, I dont have such issue but I am suffering from IBS issue where I take tablets when diarrhoea happened. Stool occult is negative. My hemoglobin is low currently at 9.8 despite taking erythropoietin injection. Kindly help and medication if possible for my IBS related. Due to my frequent loose motion, I cannot hold nutrients on my body

Probable Diagnosis:
-IBS-D (Irritable Bowel Syndrome – diarrhea predominant)
-CKD with anemia (Hb 9.8) likely EPO resistance / possible iron deficiency
Management (IBS-D):
• Tab Rifaximin 550 mg BD × 14 days
• Tab Mebeverine 135 mg BD before food
• Tab Loperamide 2 mg SOS for diarrhea
• Probiotic (VSL#3 / Darolac) BD × 2–4 weeks
• Low-FODMAP diet, avoid milk, oily & spicy food
• Increase soluble fiber (ispaghula husk at night)
Advice:
Frequent loose stools may reduce nutrient absorption → treat IBS + correct iron deficiency.
