Physiotherapy is best ans if u dont want surgery. But quality of life is not worth with conservative treatment in such case.
If acl tear is there than he will always experience instability while walking on uneven surface and climbing down the staircase. Also he cant involve himself in sports activity. Rest let ur friend take a call whether to be operated or not as eventually he is suffering who can understand the difficulty in his activities
