Namastey Based on the ultrasound findings, the most significant finding is a 19 × 11 mm well-defined lesion within the left parotid gland, which has been described as a possible cyst containing internal echogenic material. Additionally, there are a few small cervical and submandibular lymph nodes that have been reported as reactive lymphadenopathy, which generally suggests a response to inflammation or irritation rather than a serious disease process. Since the lump has been present for approximately two years without significant improvement, it is important to obtain a definitive diagnosis rather than continuing treatment solely based on assumptions. The fact that the swelling is painless, slowly persistent, and located within the parotid gland means that several possibilities need to be considered, including a benign cyst, chronic inflammatory changes, or other salivary gland conditions. The occasional deep ear pain may occur because the parotid gland and ear region share some nerve pathways.
Next Steps
As the swelling has persisted for a prolonged period and has not responded to treatment over several months, I would advise seeking a detailed evaluation by a specialist experienced in head and neck swellings. A long-standing parotid gland swelling should be properly assessed to determine its exact nature before deciding on further management. While the ultrasound provides useful information, it does not always establish a definitive diagnosis. Avoid repeatedly pressing, massaging, or manipulating the lump, as this can sometimes increase local irritation. Continue to monitor for any increase in size, development of pain, facial weakness, difficulty opening the mouth, or new symptoms. Health Tips
Maintain good hydration and oral hygiene, as salivary gland health is influenced by overall hydration status. Avoid tobacco products if applicable and follow a balanced diet rich in fresh fruits and vegetables. Since the swelling has been stable for a long duration, try not to panic, but also do not ignore it. Persistent lumps, especially those present for years, deserve proper evaluation to establish an accurate diagnosis and guide appropriate treatment. Early clarification of the cause often provides both peace of mind and the best long-term outcome. Thank you
