If it is a visible cavity and it is hurting you then please visit your dentist and get an IOPA done. After investigation get a proper diagnosis and treatment plan then either get it extracted or if possible get a root canal treatment done. Regards, Dr Vishal Jain

Based on your description probably the permanent molar is affected by cavity and extraction will not be the first recommended treatment modality.... go for root canal treatment to save the tooth, followed by a cap..... the best decision will be taken by the dentist who performs the clinical examination and takes 1 x-ray of the tooth..... Regards....

At 7 years the last tooth is either a permanent tooth which is erupting or primary second molar. Most likely if it is decayed then it should be primary molar which will need treatment. You can now give him paracetamol 5 ml and take him to a dentist who will guide you what to do.
