Koch's Infection
I am 25 male. I've had Right hip joint pain for 2 months. MRI says 'Osteomyelitis with septic arthritis, likely by koch's infection'. Advice - CT guided biopsy. I referred another doctor who asked for MT test, which came positive (20mmx17mm). doctor started Antibiotics ( R-cinex 600/ Combutol 1000).
My question is how reliable is MT test or should I go for CT biopsy to get it confirmed? If I start above antibiotics how many months do i have to take it?

Hello. MRI is a fairly reliable investigation for TB of Hip joint. Montoux test is not at all reliable and never confirmatory. The only diagnostic test is a biopsy taken open or closed aided with CT. Anyways TB is common in our country that we always start ATT on clinical suspicion and MRI alone. Keep taking ATT for 6 weeks and monitor clinical improvement. If it is there then diagnosis is almost certain.
Full course of ATT is 18 months with regular monitoring every 2 to 3 months. Never stop it midway until advised by an orthopaedic surgeon.
