Itchy scrotum, underarms, belly, head
I am 21 year old guy. I've been having these problem for last 3 years. Firstly itching starts in scrotum and between thighs and next day itching occurs in underarm and belly also including scrotum and on third day same happens along with head. When i take CZ3 tablet I get relief for next 3-4 days and after that the same problem occurs.

You are probably suffering from what Dermatologist call urticaria. Common causes are food allergy, infection or drugs taken for some condition. It presents with itching resulting in formation of red rashes which we call hives or wheals. Although in most cases cause remains unknown in spite of a thorough search.
