My nephew is 4 years old. We just noticed a few itchy black bumps in his thigh and over the penis and scrotum. What is it ? Is this a serious problem?

Thank you for a clear pic.
It appears to be some form of an Eczematous lesion.
It could well be an insect bite reaction or a reaction to some medicine taken for some other issue like a cough/cold/fever.
More history and details are required for a more clearer picture and advice especially since it's a 4 year old child.
