Itching on anus

Asked for Male, 20 Years

For piles, doctor prescribed Smuth cream, been applying it for more than a month. Piles size is reducing, but recently I've developed itching on anus and on nearby area below the scrotum. Feel like scratching the area more and more. After a few days, the area has developed flaky, dry skin on the epidermis which comes off after scratching. Doctor recommended sitz bath.
