"Severe phimosis, where the foreskin cannot be retracted at all, creates a closed environment that is highly prone to Balanoposthitis (inflammation/infection of the glans and foreskin). The intense itching you are experiencing, especially at night, is a classic sign of a fungal infection (Candidiasis) or a bacterial buildup due to trapped moisture and secretions (smegma). At age 20, if this condition is causing significant distress and sleep loss, it requires active medical intervention to prevent secondary complications like urinary tract infections or paraphimosis
Health Tips

Avoid forcefully pulling the skin back, keep the area clean and dry, Since you are describing severe phimosis with inability to retract the foreskin, it is important to consult a urologist for proper evaluation and further treatment.
