Is this thrombosed external hemorrhoids?

Hi, I consulted with a doctor on practo and shared images to help with diagnosis. Doctor said my condition is Anal fissure with external hemorrhoids. He prescribed daflon 500mg and anobliss cream with diet plan. I implemented everything as prescribed and it really reduced the hemorrhoids size upto 60%. After the prescribed course ended he asked me to continue with diet plan, which I did. Two days ago i felt some Anal itch and when I photographed it, looks like it returned. Please suggest what to do. Clicked the attached photo Today.
