It may not be due to malaria. It seems like angioedema. Mostly caused due to exposure to allergen. Can also result in itching, cold, sneezing repeatedly, fever some times.
Next Steps
Please do blood test to avoid any confusion CBC,ESR,Rapid Malaria Test, Serum IgE level
Health Tips
For instant relief can take Paracetamol thrice a day after food and levocetrizine at evening empty stomach.

Swollen lip is not a typical symptom of malaria. Malaria usually presents with fever, chills, sweating, headache, and body ache. Lip swelling could be due to an allergic reaction, infection, insect bite, or angioedema. However, since he has fever in a malaria-endemic area, he should get a malaria test (rapid test or blood smear) to rule it out.

Swelling over lip is not a sign of malaria, it can be due to allergy or insect bite or dental issues, i.e. other causes. However for intermittent fluctuating fever, malaria is a possibility. He should get tested for malaria and take treatment from some Doctor there, as per Govt. Guidelines.

Since your friend also has fluctuating fever and is in Liberia (where malaria and other tropical infections are common), he should: • Get a malaria test (rapid antigen / peripheral smear) done urgently to rule it out. • Also consult a physician or internal medicine specialist for evaluation of both fever and swelling. If the swelling increases, breathing difficulty, or throat tightness occurs — he should seek emergency care immediately (possible allergic reaction). I can guide you further with possible causes and investigations once his basic test results are available. — Dr. Yash Duragkar, MD Medicine

Lip swelling with fluctuating fever may occur due to infection, allergy, or malaria. If the lip is red, painful, or warm, a bacterial infection is likely and needs antibiotics. If fever comes with chills, get a malaria test done. Take cetirizine for itching, stay hydrated, and avoid spicy foods. Seek urgent care if swelling increases rapidly or affects breathing.
