It is a positive sign that your overall mood is stable on venlafaxine 225 mg and olanzapine 2.5 mg. However, the sudden spikes of irritation, mental arguments, and getting triggered by things people said or did in the past suggest residual emotional reactivity rather than a relapse of depression. These symptoms are commonly seen when there is underlying anxiety, rumination, poor frustration tolerance, or unresolved anger, even when mood is otherwise okay. Sometimes, antidepressants can improve mood but not fully control irritability or intrusive mental replay, which is what you seem to be describing. This does not mean treatment has failed—it means the treatment may need fine-tuning.
Next Steps
• Discuss these symptoms openly with your treating psychiatrist • Your doctor may consider: Adjusting the dose or timing of current medicines Optimising olanzapine or considering an alternative mood-stabilising option Addressing anxiety/rumination specifically • Psychotherapy (CBT or emotion-regulation focused therapy) is very helpful for mental arguments, triggers, and irritability • Practice pause techniques when irritated (delay response, grounding, slow breathing)
