Intense itchiness in the skin of Scrotum

Hi, I have an issue of intense itching on the skin of Scrotum. Especially on the top side. The skin is itching, burning, redness, and swelling. Since 5 months I am facing this problem. I consulted 2 dermatologists before 4 months, they have prescribed some antibiotics and some ointments. But, still having the same issues. There is no infection or red spots in the penis, it is particularly in the hard skin of the scrotum. When I scratch using fingers, the colour will become red and swells.
