Since my childhood i always had problems with concentration and i forget my things all the time also i jump from activity to another i can't keep doing the same activity and i keep moving and talking wich causes me many problems in school

You could start practicing mindfulness....
Begin with being present in this very moment. For eg while brushing just focus on the taste of the paste, how it feels on your tongue....the sound of the water. Just be present in this moment. If any thoughts come by just tell your mind "Be aware of this moment"
Nothing else exists...you can begin with this exercise atleast for 3 times a day.....with different activities you do and then eventually increase the frequency
