My grandmother frequently start opening her tongue out of mouth and start chewing it also sometimes,what could be potential reason as we cannot diagnose this without any clinical setup of hospital as she is not physically fit to go to doctor's clinic or hospital,if any remedy possible or not ?

What you are describing is what we call as ‘Oro-facial Dyskinesia’.
In older people this is usually due to medications they may have or are taking (like anti-psychotic medications). Sometimes older patients with dementia can also do this.
You can do the following
1. See what meditations she is taking now and also what has taken in the past few years to see if you can identify an anti-psychotic medication
2. Procyclidine 5mg twice a day for 2 weeks to see if her symptoms are better. If they are better she can continue the medication in the future. If not better discontinue.
