Went for general health check up .. In that my igE level is 1332, ESR is 69 and vitamin b12 is 203.. But I don't have any symptoms..
Recently had shoulder and neck pain doctor told to take mri .. In neck mri cervical spondylosis is there and doctor told to do physio.. In shoulder mri mild inflammation is there and doctor gave medicine for 2 weeks and if pain is there he told to put PRP injection in shoulder... My query is
1. Is this shoulder pain anyway related to igE and esr levels ?
2. What should I do further for elevated igE and esr level?
3. Is PRP safe to proceed??

Hello, I am Dr. Romain Rajan, General Physician.
I understand your concern â it is common to have incidental findings in lab tests and imaging. Your shoulder and neck pain are most likely related to cervical spondylosis and local shoulder inflammation rather than the elevated IgE or ESR
â Advices
⢠Follow the prescribed physiotherapy routine for your neck and shoulder to improve mobility and reduce pain
⢠Maintain good posture, especially during desk work, and avoid lifting heavy weights until pain improves
⢠Include a balanced diet rich in vitamin B12 or discuss supplementation with your doctor to correct the low B12 level
What to Monitor
⢠Watch for worsening pain, swelling, redness, or limitation of movement in the shoulder or neck
⢠Track any new symptoms such as fever, rashes, unexplained fatigue, or recurrent infections
Management
PRP injection is generally considered safe when done under guidance by an experienced physician and can help in persistent shoulder inflammation. Avoid self-medicating for IgE or ESR elevations. Further evaluation for high IgE may include allergy testing or immunology consultation, especially if symptoms develop
When to Consult a Doctor Immediately
⢠Sudden increase in pain, swelling, or redness at shoulder or neck
⢠High-grade fever or signs of infection
⢠New neurological symptoms like numbness, weakness, or tingling in arms or hands
Every patientâs situation is unique. For accurate diagnosis and lasting relief, it is important to discuss your medical history, lab findings, and imaging in detail
You can reach out to me directly for this case, and also connect with me in future for any medical consultation

High IgE levels only indicates some allergy, high esr is chronic inflammation, some health issue. High prp could be due to smoking, alcohol, stress, obesity, High cholesterol.
Shoulder pain would be more due to muscle strain or early joint stiffness.
Take tab flexon m.r one every 8hours for 4 to 5 days, after meals.
Get serum calcium, vit d3 done, share results.
IgE level is not as such related to shoulder pain. It just shows an allergic response or parasitic infestation. Note the allergen and avoid it. Antiallergy medications may be used to decrease it if symptomatic.
Esr indicates an inflammatory process in your body.
PRP may be employed if pain is not relieved with physiotherapy and medications. High IgE level is a sign that allergic reaction may occur. But PRP is usually safe since it's your own blood being processed and injected as platelet rich plasma thus unlikely to evoke allergic response.
Hi. IgE total is related to allergy in body. Please notice is allergy present ?
You can do cross test for igE total.
Share reports for best consultation.
Shoulder pain is not related IgE and high ESR which reflects allergic condition and some sort of inflammation in the body , respectively.PRP is relatively safe. However if your symptoms are mild ,try initially physiotherapy before prp.
I would have ignored all these findings. Everything is "mild" . It all fits well within the normal ageing process. At the best you should do your own physical exercises for the neck and shoulder. If symptoms persist for more than a month you should get a repeat MRI done to see the progress, if any.

1. Your shoulder pain is not related to IgE or ESR
both are non specific investigations I done just routinely
2. You just ignore the tests
3. It is good if your pain persists even after physiotherapy and pain killers which is not that common

Your MRI shows cervical spondylosis and mild shoulder inflammation, which are the likely reasons for your pain. These are not directly caused by the high IgE or ESR. Elevated IgE often reflects an allergic background, while ESR rises in many situations like recent infection or inflammation. Vitamin B12 is also on the lower side and should be corrected.
Next Steps
Please continue physiotherapy as advised and recheck your IgE and ESR after a few weeks. Correct your vitamin B12 with supplements. PRP injections are generally safe but should only be considered if pain continues despite medicines and physio. Since your results involve multiple areas, a direct consultation with me would allow a more tailored approach rather than treating numbers in isolation.
Health Tips
Do not self-medicate for high IgE or ESR, they are just markers and not diseases themselves. Stick to posture care, exercises, and nutrition. If pain or blood reports remain troublesome, I’d be glad to review them personally and guide you further.
Thank you for sharing your reports in detail. Let me address your queries step by step:
1. Relation of shoulder pain with IgE/ESR:
Your shoulder and neck pain, along with MRI findings of cervical spondylosis and mild shoulder inflammation, are structural and mechanical issues. These are not directly related to the raised IgE or ESR.
IgE is usually elevated in allergic conditions (like asthma, eczema, allergic rhinitis, etc.). If you don’t have allergy symptoms, it may not be clinically significant but should be monitored.
ESR is a nonspecific marker of inflammation. It can be slightly raised in many conditions, including chronic inflammation, anemia, or even age-related changes.
So, your musculoskeletal pain is more likely due to cervical spondylosis/shoulder inflammation, not IgE/ESR levels.
2. What to do about elevated IgE and ESR:
Since you don’t have allergic symptoms, extremely high IgE levels may just indicate a predisposition. Still, I recommend you see an allergist/immunologist once for further evaluation.
For ESR, it’s worth repeating the test after a few weeks, along with other inflammatory markers (CRP, CBC) to see if it persists. If persistently high, I may look for any hidden causes.
3. PRP (Platelet Rich Plasma) safety:
PRP injections are generally considered safe because they use your own blood, reducing the risk of allergic reactions. They are often used for persistent shoulder, knee, or tendon problems. The main risks are local pain, swelling, or infection (which is rare if done in sterile settings). Whether you need PRP depends on how much your pain improves with physiotherapy, medication, and exercises first. Usually, conservative treatment is tried before injections. But the call should and must be taken by the treating orthopaedic doc.
Next Steps
Next Steps:
Continue with physiotherapy as advised for your neck and shoulder.
Recheck ESR after 4–6 weeks..
Consult an allergist/immunologist regarding the high IgE, especially if you develop any allergic symptoms.
Consider PRP only if pain persists despite good physiotherapy and medical management, and after discussing risks/benefits with your orthopedician.
Health Tips
Word of Caution:
Don’t worry too much about isolated lab numbers without symptoms. Focus on lifestyle—regular exercise, posture correction, adequate vitamin B12 intake, and follow-ups. If you develop new symptoms (breathing issues, persistent rashes, weight loss, prolonged fever), inform me promptly.
