Persistent UTI with flank pain and E.coli needs proper urologic evaluation. Normal KFT/USG makes severe pyelonephritis less likely, but recurrent/partially treated infection, stones, prostatitis or obstruction should be ruled out. Avoid repeated self-antibiotics and consult with culture reports.
