Based on your description, it appears you are experiencing pityriasis simplex capillitii, commonly referred to as scalp dandruff, which may have exacerbated into seborrheic dermatitis due to the prolonged absence of scalp cleansing. The thick, adherent flakes you’re noticing are likely a combination of desquamated corneocytes, sebum, and possible overgrowth of Malassezia species, a lipophilic yeast commonly implicated in such conditions. Post-operative periods can sometimes disrupt normal scalp hygiene, leading to rapid keratinous debris accumulation, as seen in your case.
Next Steps
Start with a medicated shampoo containing a combination of 2% ketoconazole and 1% zinc pyrithione, applied to the scalp and left on for 5–10 minutes before rinsing. Use this 2–3 times a week. Additionally, a salicylic acid 2% scalp lotion or shampoo may help in keratolysis and facilitate removal of adherent scales. If the inflammation or pruritus is significant, a short course of a topical corticosteroid lotion (such as clobetasol propionate 0.05%) applied in the night can provide symptomatic relief—preferably under dermatological supervision.
Health Tips
Avoid excessive scratching as this can lead to secondary bacterial infection or folliculitis. Refrain from using hair oils temporarily, as they can exacerbate the condition by providing a nutrient-rich medium for yeast proliferation. If the condition persists despite appropriate treatment, consider evaluation for underlying conditions such as psoriasis capitis or atopic diathesis. Always follow up with a dermatologist if symptoms are refractory or worsening.
