Hyperbolic Tapering of 2 benzo drugs
Respected doctors Respected Doctors, My mother has been on psychiatric medications for the past 2 years. She is currently taking Lithium 400 mg twice daily, Clonazepam 0.5 mg, and Zolpidem 5 mg at night. She is now stable, and we would like guidance on safely tapering her medications using a gradual (hyperbolic) tapering approach. I am concerned about the risks of dependence and withdrawal, especially with long-term use of these medications. Greatly appreciate your guidance on how to safely taper with 5 percent cuts. Lithium is a very safe medication on longer run, provided regular serum lithium level test is done and other blood parameters and investigations are monitored. The issues lies with clonazepam and zolpidem both GABA drugs which are highly addictive and harmful on a longer run. Efforts can be made to taper down by an astute psychiatrist. Good luck

Ideally for tapering we have to assess for how long was the person symptomatic and what were the symptoms. If it was just one episode then after 6 months to 1 year of remaining stable we start tapering. If it was a longer illness we give treatment for atleast 1-2 years. Then tapering initially is faster and later with lower doses it's slower. Also tapering is done every 2-3 months only and not faster than that. Even If she has been on regular treatement and maintaining well a detailed history has to be taken to know what was the diagnosis and how did the disease progressed and then based on all that we can definitely try to taper off medications.
