Grade of adenoid hypertrophy? Look for signs of sleep apnea syndrome. Also see if a child is a habitual mouth breather, failure to thrive, or obese. In these cases if medical management doesn't improve the condition then surgery is a last resort.

For adenoids spray is the treatment but even then it’s not reducing then we wait for the child to be of adequate weight that is min 20kgs to take the child for surgery (adenoidectomy)
Most often the adenoids reduce with spray treatment if you use it properly
It has to be used daily twice for 6 months minimum for you to see for changes
Next Steps
Continue spray and use it regularly
Health Tips
Encourage more of physical activities to the child like skipping and running
Avoid items that give the child cold
