High fever and shivering
Father has 103 fever currently and shivering. Had meftal 500 mg. He is diabetic and already got test done for malaria, typhoid and CBC, waiting for uti culture report. What to do at the moment, which medicine to give, please advice.

Fever in diabetics at age more than 55 yrs is usually due to urinary tract or respiratory infections. Usually urinary tract infection has less dramatic but more vague symptoms.

urgent physician consultation. blood test should include urine routine examination , blood sugar, liver function test, urea creatinine and serum electrolytes. hospital admission may be necessary if persistent fever, low urine output, poor oral feeding, dehydration, low/high blood pressure. if admitted do a bllod culture also before jumping to antibiotics. ideally antibiotics should be started very early.

omit mefenamic acid (meftal). put on paracetamol 500 tds for fever. A diabetic patient is always prone to have acute kidney injury during infections
