Heavy Hairfall But Bald in Starting Stag

I'm getting heavy hair fall, My hair roots are so weaken. If I touch my head (not pulling just touch my head) leads 5 to 6 hair roots came with my hand. Day by day it becoming worst. My forehead hairs are almost gone. Middle head hairs are completely weaken. My other scenarios could help to analyse : More than I'm also facing other illness like severe cold so often. I'm also getting treatment for that. I'm living in Chennai lot of pollution Travelling daily 30 KMs for work (via public transport) I usually sleep at 2 AM to 3 AM and wake at 10 AM- 11AM due to different work time. I am totally confused, Please help me
