Take tab uprise 60,000 units once a week, for 6 to 8 weeks, Tab neurobion forte one daily for 8 weeks. For hairfall, increase proteins in your diet,  do scalp massage with lukewarm natural oil like coconut or olive oil  once or twice a week. If dandruff is present, get it treated. For heavy breathing,  get cbc, x ray chest, ecg , cardiac echo, done and share results. For relief, you can take tab acebrophylline 100mg one morning and one evening for 4 to 5 days.

Change life style n diet plan drastically first. Vitd3 Uprise d60k once a week for eight weeks followed by one per month for six months after breakfast a complete therapy by WHO. Neurokind od one after dinner for two months  for b12.

For vitamin b12 and d3 deficiency you need to take tablets.weekly.for cholesterol you need to change dietary habits first and lifestyle modifications also .take more fibrous diet and avoid oily and street food.also you need to medicine for this .HbA1c value is 5.9 its means you are prediabetic also.avoid carbohydrates as much as you can and increase excercise.
