Hair Fall & Thinning

Asked for Male, 23 Years

I am facing i tense hairfall accompanied with itching. When i itch then hair comes out with some white colour sand ball type particles at the base of the hair. Which feels as if the hair has come out with the follicles. I also face intense hair fall as well. I dont have any dandruff on my head as well. What might be the cause? Which clinic or doctor can I consult for remedies and increasing density of hair without surgical procedures?
